"Dvorak Extended" Unicode Keyboard Layout
This is a Mac OS X keyboard layout combining the attributes of the built-in Dvorak and U.S. Extended layouts. It allows you to type the extra Unicode characters from U.S. Extended (such as ē, ʉ, and ǽ) without having to switch your keyboard (and brain) back to QWERTY. It was made with Ukelele, the freeware keyboard editor, and while it seems to have a few quirks (i.e. keyboard shortcuts aren't working right in one old-ass version of Photoshop), it generally works pretty well.
To install, you'll need to put both Dvorak Extended.keylayout and Dvorak Extended.icns in /Library/Keyboard Layouts or /Users/[you]/Library/Keyboard Layouts. You'll then have to open the International pane in System Preferences, switch to the Input Menu tab, and enable Dvorak Extended in the list of layouts and input methods. (You may also want to log out and log back in, because I ran into some weirdness when I first turned it on.) After that, you'll be able to select it from the input menu along with the rest of your keyboard layouts.

(P.S.—Ukelele is amazing. I just used the Swap Keys... command over and over again, but you could use it to make practically anything. If you ever have to do something unholy involving text input, give it a try.)
